Required Immunizations and Health Assessments
East Hampton Public Schools

Based on guidelines provided by Connecticut General Statutes, the East Hampton Board of Education requires that a formal health assessment be conducted prior to initial enrollment, while in grade 6 and while in grade 10. Grade 6 health assessments may be conducted anytime after the completion of grade 5 but prior to entering grade 7. Students who have not submitted proof of a health assessment to the school by the completion of the 6th grade will be provided a thirty (30) calendar day notice, in writing, prior to the effective date of school exclusion.

Grade 10 health assessments may be conducted anytime after the completion of grade 9 but prior to entering grade 11. Students who have not submitted proof of a health assessment to the school by the completion of the 10th grade will be provided a thirty (30) calendar day notice, in writing, prior to the effective date of school exclusion.

School health assessments are required by state law and represent a simple procedure that maintains the health of our students. The minimum health assessment includes a hematocrit or hemoglobin test, recordings of height, weight and blood pressure and gross dental, postural, vision and auditory screens. State law also provides an examination from this requirement should a parent, guardian or student object on religious grounds.

Proof of medically appropriate immunizations shall be required prior to entering the school system. Unless exempt by state law, all new school entrants must present completed documentation of adequate immunizations against measles, mumps, rubella, Hemophilus type B (one dose at 12 months or older for all children under five years old), poliomyelitis, diphtheria, tetanus-pertussin, varicella and Hepatitis B.

All students entering kindergarten must show proof of having received two (2) doses of measles containing vaccine. All students entering 7th grade must also show proof of having received two (2) doses of measles containing vaccine.

In addition, all students entering kindergarten who are 48-71 months of age must show proof of having received at least four (4) doses of DTP/DTaP vaccine and three (3) doses of polio vaccine (IVP/OPV) with at least one dose given on or after the 4th birthday and before school entry.

All students are required to show proof of immunity to varicella (chickenpox) before entry into 7th grade. All students born December 31, 1996 or later must show proof of immunity to varicella for entry into licensed pre-school programs and kindergarten.

Proof of immunity to varicella includes any of the following:

1.  Documentation of age-appropriate immunization (one dose given on or after the student’s first birthday if they are less than 13 years old, two doses given at least 30 days apart for students whose initial vaccination is at 13 years of age or older), or
2.  Serologic evidence of past infection confirmed in writing by a certified lab, or
3.  A statement signed and dated by a physician indicating that the child has already had chickenpox based on family and/or medical history.

In addition to documentation of chickenpox immunity, all 7th grade students must show receipt of at least one (1) dose of Hepatitis B vaccine or show proof of serologic evidence of infection with Hepatitis B. Students must also show proof of three (3) doses of Hepatitis B vaccine or serologic evidence of infection to enter 8th grade. This does not change the requirement that all children born after January 1, 1994 must have three (3) doses of Hepatitis B vaccine or serologic evidence of infection before entering kindergarten.

Failure to comply with the immunization requirements will result in exclusion from school. Unlike health assessment requirements, there is no grace period with immunization requirements. However, exceptions and exemptions from the immunization requirements are provided within the state statutes and the Board’s policy.

East Hampton students who intend to participate in interscholastic sports are reminded that they are required to obtain annually a sports physical examination before their first practice. To be valid, the physical examination must have been completed within one year of the ending date of the sport and be on record in the health office of the school attended by the student.
