The following items must be received by Center Elementary School from the parent/guardian PRIOR TO THE FIRST DAY OF SCHOOL (all grades). The student will NOT be permitted to begin his/her first day until ALL materials have been received by the school office. If you have any questions regarding these requirements, please call the school office at 860-365-4050.
 State of Connecticut Health Assessment record (blue form—both sides). Proof of immunization (see attached). Birth certificate indicating mother and father. (Connecticut wallet-sized birth certificate is not acceptable.) Proof of primary custodial parent’s residency in East Hampton—copy of tax bill, utility bill, phone bill, lease agreement or closing paperwork. (Connecticut driver’s license is not acceptable.) Child’s last report card from his/her previous school, if available. Release of records form to allow us to obtain information from the student’s previous school. Registration Form. Transportation Request Form. Copy of the student’s cumulative record from the previous school. ALL special education reports, minutes, I.E.P. reports, etc., are requested. | |
